SUBMITTING A PROPOSAL
Proposals should be prepared using the following outline as a guide.ABOUT THE PROJECT:
- description of the project and what it may be expected to achieve
- description of the people, organizations or groups expected to benefit from the project’s outcome and the ways they would benefit
- description of the plan of action and a timetable for implementation
- method and criteria for assessing the project’s effectiveness
- qualifications, experience and affiliations of people involved in implementing the project
- detailed expense budget for the project indicating how the requested funds are to be spent
- description of other possible sources of support which have been or will be solicited for the project
An applicant will be notified shortly after a decision has been reached. Funds will be sent to the grant recipient in accordance with the approved budget and any conditions the Foundation establishes. Funds are forwarded based upon evidence of expenditure in the form of progress payment reports, invoices or similar documentation.
ABOUT THE APPLICANT ORGANIZATION:
- legal name and address
- name, title, address and telephone number of the person charged with the responsibility for the application
- description of the organization and its qualifications in the area for which funds are sought
- a list of names and primary affiliation of the organization’s Board of Directors
- statement of the organization’s financial status, preferably an audited balance sheet and income statement for the most recent fiscal year
- copy of the most recent ruling from the Internal Revenue Service stating that the organization is tax exempt under section 501 (c) (3)
- statement from the organization’s chief operating officer that the project has been reviewed and recommended for submission to the foundation.
